For providers it means better support and time to focus on patients, and for partners that means higher quality and lower cost.Join us in our mission!SUMMARYThe Housing Navigator (HN) is responsible for providing support and assistance to members and their families experiencing homelessness or at risk of losing housing.WHERE YOULL WORKThis position is hybrid. Work from home and go into the field (Orange County).HNs are required to travel to members within designated areas. You will have full control over your schedule when meeting members. You are eligible for mileage reimbursement for the use of your vehicle for business-related travel. Standard business hours are Monday through Friday from 8:30 am to 5:00 pm.
WHAT YOULL DOResearch, establish, and maintain a list of available housing opportunities in the regionDevelop relationships with and become knowledgeable about local community resources including transitional housing, shelters, and permanent housingDevelop individualized housing plans and communicate plans and housing opportunities to membersEnsure effective service and all placements are consistent with member care planAssist members with the housing search, including applying for apartments, negotiating with potential landlords, and filling out rental applicationsAssist members in understanding tenant rights and responsibilitiesAssist members with landlord communications by being a mediator if there are disagreements or concernsConduct follow-up meetings with members after housing placement to assist with any issues that may have arisenMaintain accurate documentation of service objectives, outcomes, and other member-related information in appropriate data systemsParticipate in collaborative team meetings to provide input, support, brainstorm solutions to common areas of concern, and develop best practicesUnderstand and abide by all departmental and companywide policies and proceduresComply with all safety and injury prevention policies and regulationsUtilize the Homeless Management Information System (HMIS) to add members into housing portalAssist members complete the CES assessmentsAssist members in-person with assessments or housing tenancyPerform other duties as assigned or required per departmental policy
WHO YOU ARESensory RequirementsFluent in English (written and verbal), Bilingual in Spanish
Ability to communicate clearly in-person, by phone, and electronicallyAdequate hearing and vision (with corrective devices if necessary) to conduct assessments and documentation

WHAT YOULL NEEDDiploma/GED required, associate degree preferredMinimum 2+ years of experience in healthcare, social services, public health,case management, or working with unhoused and unsheltered individuals, families, and veteran programsPrior experience working with HMISPrior experience working in a social service setting with working knowledge of case management systems and planning techniquesCurrent and valid Drivers License with a clean driving recordReliable transportation and proof of auto insuranceCurrent BLS certification from the American Heart Association upon start dateDistraction-free home workspace with a secure internet connection
