Description:

Introduction: We are seeking an organized and compassionate Case Manager to join our team and provide support and guidance to clients in need. The Case Manager will be responsible for working with clients to identify their needs and goals, developing and implementing action plans, and providing ongoing support and monitoring to ensure progress. The successful candidate will have strong communication and problem‑solving skills and be able to work collaboratively with other team members.

Responsibilities:

Work with clients to identify their needs and goals, including conducting assessments and gathering relevant information

Develop and implement action plans to help clients achieve their goals, including coordinating with other service providers as needed

Provide ongoing support and monitoring to ensure progress, including conducting follow‑up meetings and check‑ins

Maintain accurate and up‑to‑date records of client interactions and progress

Collaborate with other team members to ensure continuity of care and services

Other duties as assigned

Qualifications:

Bachelor's degree in social work, psychology, or a related field

Minimum of 2 years of experience as a case manager or in a similar role

Strong communication and problem‑solving skills

Ability to multitask and handle a high‑volume workload

Ability to work collaboratively with other team members

Valid driver's license and reliable transportation are a plus
