Family Medicine NP/PA: Flexible Hours, Collaborative Care

Community Care Physicians – Community Care Family Practice Colonie is seeking a Nurse Practitioner or Physician Assistant to join the Colonie practice, delivering full-scope family medicine with strong physician support.

The Advanced Practitioner will provide preventive care, manage acute and chronic conditions, order studies, prescribe medications, and coordinate referrals within a collaborative team. NYS licensure and active registration are required, with ambulatory care experience preferred.
