At-Home Physical Therapist: Boost Mobility & Independence

CenterWell Home Health seeks a dedicated Home Health Physical Therapist to deliver in-home therapy, improving functional outcomes and helping patients regain independence in daily living. Reporting to the Clinical Manager, you will assess patients, plan rehabilitation, supervise PTAs and aides, and document progress per physicians' plans.

This is a full-time role requiring an active PT license and CPR certification.
