RESPONSIBILITIES

Patient Care. Take complex medical histories on patients referred for consultation by academic and community physicians; perform physical examinations; order laboratory tests, x-rays, special imaging studies and other diagnostic procedures

Management & Development. reach tentative diagnosis of medical, surgical and other conditions; develop treatment plans in conjunction with attending physician to provide optimal patient medical management; order consultations and interact with members of other specialty medical teams; assist in the collection of data for research projects

Patient Satisfaction. Interacts on a frequent, regular and daily basis with patients, family members, medical/social/public services support, regulatory agencies and/or payor agencies. Interaction and communication requires expressed empathy, understanding, problem-solving, education, information gathering, process influencing and counseling.

Medication & Orders. Prescribe medication, write orders, perform procedures and respond to emergency and/or immediate condition changes of acute patients.

Documentation. Obtain complete, detailed and accurate patient histories, review patient records, perform physical exams, record all pertinent data to develop comprehensive medical status, diagnosis and clinical treatment plan for all assigned patients.

Job Overview This position provides nursing services, either directly or indirectly, to participants in a specific program, specialty department, or practice. The role includes initial assessments, regular re-assessments, ongoing nursing services, interdisciplinary collaboration, prevention, early detection, acute and chronic illness care. Inpatient RN roles may include verbal orders, order entry, and pend access notes, and require attendance at physician THC training.

Job Responsibilities

Plan care for participants and family based on assessment, standards of care and optimal outcomes. Initiate and individualize appropriate patient care guidelines, POC or clinical pathways. Maintain current plan of care by updating as needed. Develop both short- and long-term goals. Participate as a member of the Interdisciplinary Team by identifying caregiver and participant needs, recommending interventions and collaborating with other health care team members for mutually determined solutions.

Perform technical skills according to hospital's and department's policies/procedures, appropriate competencies, and accepted standards of practice. Implement POC. Prioritize workload according to participants' needs and requests. Base interventions on clinical data, desired outcomes, the nursing process, and standards of care. Document interventions and outcomes. Utilize critical thinking skills in daily care, including accurately receiving, interpreting and executing physician orders.

Evaluate the plan of care based on optimal patient outcomes. Document the participant / family response to care including teaching. Collaborate with other members of the health care team to revise plan of care as needed. Supervise the care that was delegated to other health care team members.

Demonstrate organizational responsibilities: Identify areas for self‑improvement, function in a relief charge/resource role as requested, complete assignments within scheduled timeframes, cooperate with instructor to facilitate effective learning experiences for students, maintain current knowledge in area of practice, demonstrate knowledge of organizational and unit changes.

Conduct and document comprehensive patient assessments with emphasis on promotion of health, prevention, and early detection to plan and implement care directed toward optimal specific outcomes. Include health counseling and teaching needs in assessments. Communicate assessment findings to other health care providers as appropriate.
